NHA Application Requirements

You can apply with the NHA here.

The classification offered by the NHA is the Certified Phlebotomy Technician or CPT title.

To take the certification exam that is offered by the NHA, an individual must show that she has work experience or an approved education in phlebotomy.

All applicants requesting certification through the NHA must apply online. Paper applications are not accepted. Candidates will need to create a profile.

Phlebotomy Certification with the NHA

This profile includes:

  1. The Candidate’s Name
  2. Address
  3. Phone Number
  4. Date of Birth
  5. Primary Language

Once the profile is complete, the candidate can register and then apply to take the phlebotomy certification exam. The cost to take the exam is $105. You will need a scaled score of 390 to pass the exam (NHA score scale: 200-500).

The NHA offers its candidates the ability to purchase study guides to assist them in preparing for their certification tests. There is a specific study guide available for those phlebotomists who will be practicing in California. This study guide costs $40 whereas; the cost of the guide for all other phlebotomists is $56.

Members are required to recertify every two years and required to complete 10 continuing education credit hours between renewals to be eligible for recertification. The recertification fee is $79.50.

The NHA does not offer a CPT examination outline. However, the test is likely to resemble the certification test offered by ASCP and AMT.
